Nurse Specialist Non-Malignant Haematology

The post holder will provide clinical leadership and responsibility for the day to day management, delivery and development of the non-malignant nursing service at Alder Hey Children’s hospital.
You must be a qualified children’s nurse with experience in caring for patients with these conditions and a particular interest/focus will be on children with sickle cell disease, thalassaemia and rare inherited anaemias.
Create and develop an individual care plan for each affected child with non-malignant haematology
Carry out assessment of the health and social needs of individuals with major disease in a timely manner and make appropriate referrals.
Participate in the planning and implementation of packages of care to meet those needs and provide individual outcomes and end of year reports for the whole service.
Undertake nurse-led clinics (mainly hydroxycarbamide monitoring) acting autonomously to provide a safe and seamless service for provision of care.
Carry out home and school visits.
Act as the first contact for all affected children and collaborative joint working with the health visiting teams, school nurses etc., the hospital day services and co-ordination of multi-disciplinary teams and clinics.
Work with young affected individual children to help them to manage themselves and reduce the impact of the illness on their lives, to help them to reach their full potential.
Contribute to the development of a transitional service for this client group, maintaining strong links with the adult haematology service.
Participate in learning programmes to pre- and post registration nurses
Comply with Clinical Governance Frameworks of the Trust.
Facilitate and assist with any audit or research being undertaken in the centre or with partners.
Participate in quality dashboard and CQUINS , and Peer Review
Support and line manage junior nurses within the team.

JOB DESCRIPTION

Post: Nurse Specialist Non-Malignant Haematology (NMH)
Band: 7 (or Band 6 initially depending on experience)
Hours: 37.5
Accountable to: Associate Chief Nurse.
Reports to: Lead Nurse for Cancer Services
Responsible for : Providing Nursing Care to Children with Non-Malignant Haematological conditions.

JOB SUMMARY:

The post holder will provide clinical leadership and be responsible for the day-to-day management, delivery and development of the non-malignant haematology (NMH) nursing service at Alder Hey Hospital.
The post holder will be a qualified children’s nurse with experience in caring for patients with these conditions and a particular interest/focus will be on children with sickle cell disease, thalassaemia and rare inherited anaemias.
The post holder will hold a key role within the Multidisciplinary Team to develop practice and services.

THE ROLE WILL INVOLVE:

Provision of care to children NMH in the community and in the hospital setting to maintain standards and equity of care.
Comply with national and local guidelines or standards for the management of children with NMH according to peer review guidance
Participate in nurse led clinics.
Develop protocols and standards for home and hospital care, to facilitate safe practice.
Collaboration and joint working with acute services to improve the quality of hospital care to improve outcomes and provide outcomes.
Support individuals with major disease and their families to reduce the burdens associated with chronic illness.
Provision of information and health education to enable participants to make informed decisions about their lives; by undertaking genetic counselling and screening of individuals and families at risk of NMH.
Participate in pre and post registration teaching to health and social care professionals.
Participate in outreach work to raise awareness in the community.
Liaison with other agencies such as the safeguarding team and social care.
Involvement in team building and service development.
Regularly review information materials provided for clients.

TO BE RESPONSIBLE FOR:

Undertaking of duties in dealing with affected individual child in the most appropriate settings in accordance with parent wishes and young person’s as appropriate.
Maintain regular communication with paediatric consultants and specialist paediatric nurse, attending paediatric haematology clinics as necessary to ensure that there is a joined-up approach in care planning between the hospital and the community.
Work in partnership with the paediatric teams and services within the trust, the haematology team and ward staff to support patients within the hospital setting.
Be involved with the functioning of the HCC and provide specialist support, education and advice for the LHT.
Contribute to the development of a transitional service for this client group, maintaining strong links with the adult haematology service.
Raising Awareness through: Involvement in community health events, giving talks to community groups and parents of affected children.
Planning and participating in multi-disciplinary study days for health and other professionals.
Developing partnerships with other agencies to support children with major disease in the community.
Working with other team members to deliver service and wider Trust objectives
Keeping accurate records in line with Trust and Nursing and Midwifery Council’s Policies.

Provide cross-cover for the other haematology nurse specialists within the service as required.

Carry out any duties which the service manager considers necessary to meet the objectives of the service and the Trust.
Supervise others in the service in the absence of senior colleagues
Work in accordance with financial management goals to ensure a balanced budget.
