Position Location: St Dominic Hospital - Outpatient Rehab
Position Schedule: Part-Time M-F

Pelvic Health experience preferred. The Occupational Therapist 1 is responsible for evaluating/assessing needs of referred patients and formulating treatment plans. Provides therapy services defined in treatment plans. Works cooperatively with physicians, case managers, and other members of the healthcare team. Maintains accurate documentation of therapy services in the medical record. May supervise certified occupational therapy assistants, therapy techs, and students. The position relies on education and training to complete responsibilities. Initially, it requires instruction and guidance to accomplish the job and should develop into working under general supervision.

  • Evaluation and treatment methods appropriate to diagnosis, age group, patient’s physical and emotional status are performed and accurately documented and appropriate treatment plan is formulated as evidenced by chart review.
  • Patient care is documented daily utilizing approved hospital and department documentation system reflecting patient response to treatment, actual performance, and treatment summaries if needed as evidenced by accurate & timely chart reviews.
  • Daily patient charges are completed accurately, recording in treatment documentation, on daily charge sheet of department, as well as entering charges in appropriate billing system per unit 100% of the time.
  • Measureable short and long term goals for each patient evaluated are identified and documented as evidence through chart review.
  • Patient and family/caregiver education concerning treatment plan, expected reactions to treatment, safe use of equipment, home programs, and follow up care is accurately documented 100% of the time.
  • Discharge notes that include a summary of the course of treatment, progress toward goals, discharge needs, recommendations for follow-up, and patient or family education are accurately completed 100% of the time as evidenced by chart review.

  • Actively contributes to educational efforts of department ( in-services, students/ new hire orientation, QI) is provided as evidenced by 1 in-service per year, documentation on competencies for students and new hires and QI data collection committee participation as assigned.
  • As a clinical instructor for Occupational Therapy students on clinical internship provided mentoring, supervision, timely feedback and written evaluation of student’s skills as evidenced by formal/written feedback/evaluation not less than twice during the student’s internship.
  • Effectively follows and participates in scheduling functions by promptly accommodating patients for treatment as ordered and effectively utilizing support staff for maximum productivity and with fair distribution at all times.
