ABOUT US:

The Transition Care Program (TCP) provides short-term care that seeks to optimise functioning and independence of older people after a hospital stay. TCP is goal-orientated, time-limited and therapy focused. It provides older people with packages of services that include low intensity therapy, such as physiotherapy and occupational therapy as well as social work, nursing support or personal care. It seeks to enable older people to return home after a hospital stay rather than enter residential care prematurely.

VACCINE PREVENTABLE DISEASES (VPD) REQUIREMENTS:

It is a condition of employment for this role for the employee to be, and remain, vaccinated against vaccine preventable diseases as listed on the Role Description.

Responsibilities:

The Occupational Therapist works within the Transition Care Program (TCP) in community settings according to Commonwealth guidelines and the Aged Care Act 1997, bringing an Occupational Therapist focus to the multi-disciplinary team and to client care, and case managing frail aged clients to appropriate service providers.

  • Case Manage clients for service provision while they are on the Community Rehabilitation Program.
  • Provide Clinical Rehabilitation Therapy to clients who are on the Community Rehabilitation Program.
