Description:
About the role
We are seeking a full-time Outreach Case Manager for our Hume Program, which provides outreach case management to individuals and families facing homelessness or at risk of becoming homeless. This is done through a team case management approach, where all team members contribute to the case plan and ongoing support for the consumer. Our goal is to enhance consumer outcomes by providing additional support alongside the existing services offered by Access Points under the Opening Doors Framework.
We are seeking someone with excellent case management skills, and familiarity within the Homelessness sector would be advantageous.
