Description:
PART TIME ROLE - 15 HOURS A WEEK (Monday - Friday - days to be discussed)
This is an exciting opportunity to join an established team.
The post holder will be based at Stepping Hill Hospital and will be responsible for contacting patients to offer them the option of transferring to an Independent Sector Provider for continuation of their care.
The post-holder will be required to use computerised systems such as Patient Administration System (PAS), Microsoft Excel, Advantis, and Pathway Monitoring System (PMS).
To work as a member of the Directorate’s Administration Service ensuring that a high quality and efficient service is provided at all times, which may include giving support and providing cover in other areas as requested.
To telephone patients to explain the process and obtain consent for transfer to the independent sector.
To ensure the timely and accurate transfer of patient pathways to the independent sector providers using trust information systems.
To monitor centralised email inbox to ensure a prompt response to enquires.
Answer general telephone enquiries from both staff and patients regarding independent sector transfers.
Input, monitor and manage patient information on various IT systems
Discharge episodes on PAS once patient has been transferred and update RTT status
