Description:

As a paediatric nurse you will be working autonomously to undertake Health Needs Assessments of children and young people to enable you to provide individual health plans / care plans to support the teaching staff to meet the continuing health care needs of the CYP in the school setting.
Working in partnership with school and other professionals involved in the young person’s care and with their parents or carers to support their continuing health care needs within school.
You will be responsible for planning and delivering teaching sessions to education staff to ensure they are trained and competent to meet the health needs of the children and young people.
You will be responsible for undertaking Review Health Assessments for Looked After Children and contribute to Multi-disciplinary Team (MDT)meetings as required.
