CLCH Hertfordshire Adult Community Services - CLCH NHS Trust provides a range of Community Services, and the Hertfordshire Division is recruiting!
We are expanding our Parkinson’s Disease service in South and West Hertfordshire, and have an exciting opportunity for a new Specialist Nurse in Parkinson’s disease. This could be the role for you.

This role involves providing care for people with Parkinson’s within the community, including running out-patient clinics and visiting people in their homes. You will be involved in medication management, and nurse prescribing training will be available if required.
You will be working alongside our current Parkinson’s Nurse and be supported by our great multi-disciplinary team environment, alongside many highly skilled and experienced Physiotherapists, Specialist Nurses, Occupational Therapists, Psychologists, support staff and will also have support from a Consultant.
The service is passionate about maintaining high standards of care. You will be involved in contributing to the improvement of services and ensuring quality care provision. As a highly specialist role, you will have opportunities to show leadership within the Parkinson’s service.

To work autonomously to deliver specialist community based nursing care for patients with Parkinson’s, based on principles that are evidence based and which enhance the quality of patient care.
To develop and deliver a specialist community based service for patients with Parkinson’s Disease (PD) acting as an expert resource in PD by working closely with patients, carers, consultants, general practitioners, therapists, social services and other stakeholders across primary and secondary care.
To be a resource and provide specialist advice, education and support to other health care personnel and people with Parkinson’s, and contribute to planning, delivering and evaluating programmes of education and development for Parkinson’s care
Work with the specialist teams to develop and implement service that is NICE guidance compliant, and which supports multi-disciplinary teams caring for PD patients by providing education and training to multi-disciplinary teams in the best practice management of PD.
