Description:

PATIENT CARE MANAGER (NURSE), PRIVATE PRACTICE, ROODEPOORT

A specialist Physician in Roodepoort is looking for a patient care manager who can plan, direct and coordinate all the medical activities of the Practice relating to in-and out-patients. The primary responsibility is patient care, patient safety, and creating systems and protocols. The role involves providing additional patient care and administrative support to the specialist. The candidate must have a nursing qualification together with management experience.

QUALIFICATIONS

  • Bachelors degree in Nursing
  • Registered nurse license
  • Registered with the HPCSA
  • Minimum of three years in a nursing role
  • Previous nursing experience in a doctors practice is advantageous

    Monday to Friday: 08h00 17h00
    Required:

Responsibilities:

  • Medical management of all in and out-patients.
  • Monitor, coordinate and implement patient treatment plans ensuring these are executed.
  • Patient education and support on the diagnoses/ prognoses.
  • Providing guidance and assistance on treatment plans, regular check ins with patients regarding health status and wellbeing.
  • Post hospitalisation check-ins.
  • Management and identification of redflags in the medical results (pathology, radiology, and any other diagnostic test results) of patients and notification to Doctor.
  • Ensuring compliance with all applicable laws, regulations and industry standards related to patient care.
  • Implementing and ensuring compliance with standards of nursing practice which promotes health care delivery.
  • Identifying and responding promptly and appropriately to legal or risk management patient care issues.
  • Medical consumable stock management for nursing station.
  • Administrative duties medical insurance claim forms, chronic illness applications, letters of motivation, preparation of prescriptions etc
  • Management of infection control in the practice and implementation of infection control protocols and safety standards in the practice.
  • Up-to date with new medicine regimens and medicines released on the market.
  • Meeting with pharmaceutical companies representatives in order to keep up to date with newest medications available in South Africa.
  • Creation of patient education pamphlets for chronic conditions.
  • Creation of standard dietary pamphlets for chronic conditions.
