To ensure that all stages of the patients journey through the EmergencyDepartment are timely, and in accordance with Welsh Government (WG)Emergency Unit targets, considering the Clinical Exceptions (Stop Clocks) Frequent liaison with the Nurse In Charge, who will establish and communicate clinical priorities. Will assist in setting actions and expectations for each patients management, which are monitored and amended as required on an hourly basis, liaising closely with the departmental Nurse in Charge. Operate agreed escalation processes to remove both internal and external barriers to the achievement of the waiting time target.
Support clinicians in the delivery of the Internal Professional Standards (IPS) to support the patient journey. Work collaboratively with internal and external agencies, to produce data and information relating to performance. Develop effective and innovative communication strategies to disseminateinformation relating to patient flow management and performance. Will develop audit tools, collate data and participate in regular audits in relation to patient flow, presenting data at relevant forums. To maintain and protect confidential information relating to patients at all times. Be responsible for expediting the transfer of patients from the EmergencyDepartment to wards To communicate and negotiate with the Nurse in Charge of each area of the ED to expedite efficient patient flow throughout the UHB. Identify any patient safety concerns in line with UHB policies and risk strategies, ensuring this is communicated to all relevant clinical staff within UHB
