Description:
Unaccompanied asylum-seeking children (UASC), arriving in the UK, have significant mental and physical health needs – many of which remain unmet by currently available services. To address this, an ‘integrated pathway’ for UASC was set up in Newham. We are now looking for enthusiastic and dynamic team members to work on a new pilot project across North East London to develop this integrated pathway.
To be the point of contact for patients for all issues relating to their pathway of care. The post holder will work as part of the service, under the direction of the Operational Lead, and will provide expertise in the proactive management of the patient pathway from referral to discharge, ensuring the entire pathway is managed smoothly.

To provide a consistent approach across the Trust.
