Unaccompanied asylum-seeking children (UASC), arriving in the UK, have significant mental and physical health needs – many of which remain unmet by currently available services. To address this, an ‘integrated pathway’ for UASC was set up in Newham. We are now looking for enthusiastic and dynamic team members to work on a new pilot project across North East London to develop this integrated pathway.
To be the point of contact for patients for all issues relating to their pathway of care. The post holder will work as part of the service, under the direction of the Operational Lead, and will provide expertise in the proactive management of the patient pathway from referral to discharge, ensuring the entire pathway is managed smoothly.

The post holder will ensure high levels of patient and clinician satisfaction by being an accessible, customer focused and knowledgeable point of contact. To provide a consistent approach across the Trust. The Standard Operational Policy (SOP) will describe how the functions of this role will be carried out along with the timescales that will be adhered to
