Description:
MINIMUM QUALIFICATIONS:
- Requires a High School Diploma/GED and a Certificate from a state approved Peer Support Training Program.
- A minimum of one (1) to three (3) years’ experience in a behavioral health setting, required.
- An individual who is, or has been, a recipient of behavioral health services (self-disclosure).
- Must complete training and demonstrate competence in Integrated Care skills.
- Must have demonstrated experience in Microsoft Office applications.
- Satisfactory work records as determined by the People 1st
- Successfully completes a pre-employment drug screening.
- Complete all on-boarding paperwork, in a timely manner and be cleared to attend New Employee Orientation (NEO), as scheduled.
- Ability to obtain and maintain a Level One Fingerprint Clearance Card and any other required background checks.
- Ability to meet Copa’s driver authorized driving requirements, set forth by Copa Health’s insurance company. Including AZ Driver’s license upon hire, 39-month MVR, proof of insurance and vehicle registration, if applicable
JOB SUMMARY (INCLUDING, BUT NOT LIMITED TO)
- Participate in morning meetings, staffing and other assigned meetings with participants, family members and collateral agencies to support assigned participants in expressing their perspective.
- Partners with NT19 Connective Care Coordinator to provide coverage support when needed or assigned.
- Work with an assigned caseload of NT19 participants to develop and achieve specific ISRP goals.
- Assistance in identifying goals in the areas of living, learning, working, wellness, and self-advocacy.
- Assistance in implementing ISRP goals by providing transportation, support in setting appointments, debriefing successes, and next steps, coaching through periods of frustration, and identifying natural and family-based supports and community resources that enhance self-efficacy and self-sufficiency.
- Meet with participants in the hospital/jail/ASH.
- Conduct hospital discharge follow-up.
- Participate in outreach, engagement, and re-engagement with a focus on community-based re-engagement.
- To ensure compliance & adequate services, additional job duties may be required to meet the needs of the program and or department, consistent with Copa Health productivity levels
