MINIMUM QUALIFICATIONS

• Bachelor’s degree in Pharmacy required.
• Current Ohio license as a pharmacist by the Ohio State Board of Pharmacy required.
• Current registration as a pharmacist by the Ohio State Board of Pharmacy required.

• Responsible to Pharmacy for assuring that the drug therapy needs of the patient are met.
• Responsible to the customer for courteous, accurate, timely service.
• Provides pharmacy services to optimize pharmaceutical care, provides drug information to patients/health care professionals, exercises applicable legal and professional controls for controlled substances maintained/dispensed, meets administrative requirements of the department, and performs other related duties as assigned
