PAY RANGE: $51.00-$84.00 HOURLY

Job Summary
The Pharmacy Medication Safety Coordinator is responsible for all aspects of medication safety programs and tracking at Fairfield Medical Center. In addition to medication safety, the scope of the position will also include oversight of regulatory standards related to medication safety and Pharmacy performance improvement. This involves coordination with the Medication Safety Officer to refine and enhance medication safety processes and accomplish performance improvement initiatives set forth by Fairfield Medical Center.
The Medication Safety Coordinator also provides pharmaceutical care to patients including monitoring, evaluation, and assurance of the quality of medication use, provision of drug information, distribution, and control of drugs and other patient services. Pharmacist staffing duties expected to be 25% of total time worked, but can vary based on departmental needs.
Performs all duties and responsibilities in accordance to the established FMC policies, procedures and guidelines.
Education: Bachelor of Science or Pharm D. degree from an American Colleges of Pharmacy Education required. Must have a valid Ohio Pharmacy License in good standing with the Ohio Board of Pharmacy.
Experience: 2 years of hospital pharmacy experience
Job Expectations and Responsibilities:

Operational Duties:

  • Assesses all aspects of the medication safety processes, including the assessment of activities, development of curricula to meet the needs, implementation of programs/offerings, and the outcomes of such programs/offerings.
  • Develops, reviews, and revises procedures, both directly and on a consultation basis, safety records, performance checklists, orientation checklists, patient education materials, etc., as they relate to staff/patient safety, including assisting in orientation and all applicable continuing education.
  • Acts as a resource to FMC clinical instructors on medication safety initiatives, teaching strategies, and continual evaluation in clinical care environments
  • Coordinate with Pharmacy Manager regarding regulatory requirements and development of educational materials for Pharmacy and all hospital staff related to gaps in medication management practice standards.
  • Responsible for follow-up and communication regarding medication events, particularly at daily Pharmacy huddles and at the weekly Medication Event Workgroup
  • Production of quarterly medication safety publication for pharmacy, nursing and other clinical departments regarding trends in medication safety opportunities and educational offerings at FMC.
  • Coordinates with Medication Safety Officer and Pharmacy Leadership at monthly Medication Safety Committee to organize overall strategy related medication safety initiatives
  • Performs other duties as requested.

Clinical Duties

  • Reviews, interprets, processes, and validates and/or dispenses medication orders and prescriptions accurately.
  • Monitors and manages patient medication therapy to ensure therapeutic appropriateness and effectiveness of drug regimens, including appropriateness to the age of the individual patient.
  • Maintains established departmental policies and procedures, objectives, quality improvement program, and records consistent with regulations and standards governing the practice of pharmacy.
  • Communicates drug information to patients, family, and other health care providers.
  • Participates in drug therapy management of medical emergencies by responding to emergency call, assisting in medication preparation and provision of drug information and assuring adequate medication supplies.
  • Coordinates the day-to-day pharmacy technician activities to meet the needs of the patient, organization and department.
