Description:
Clinical Clinical assessment & triage of patients to determine eligibility to participate in Falls Prevention: Community Exercise programmes and classes Taking escalation requests from volunteers and partner staff, e.g. GPs, Nurse and clinically advising on safety, safeguarding and ongoing action to protect the safety and wellbeing of patients and volunteers Running pre-designed group exercise classes, supported by volunteers in a range of clinical settings GPs surgeries, community halls, Nursing Homes Training and advising clinical staff in partner organisations to promote the Re Conditioning Games and advise on ways to prevent elderly service users from de-conditioning in the community and nursing home sector Provide 121 and group clinical supervision to volunteers and clinical staff in partner organisations Promoting the End PJ paralysis initiative on the wards, managing a team of students Service development Identify and build partnership relationships with an SLA or similar to enable the delivery of the Falls Prevention: Community Exercise Programme in Nursing Homes, GP practices and pre-operative assessment (Kingston Hospital). Adapt the existing service model to suit new environments of care in the community; ensure SOPs, Delivery Manuals and appropriate documentation are in place to run these services effectively Recruit and train volunteers to support the delivery of these services in the community on both a 121 (pre-operative assessment / GPs) and class based model (GPs/Nursing Homes) Establish clear referral pathways to enable patients to be referred into the Falls Prevention: Community Exercise services Write DPIAs to oversee the information governance and data sharing arrangements with GPs surgeries as necessary for referring into the Falls Prevention: Community Exercise Volunteers programme Develop an Insight & Impact evaluation model, including 8-week patient follow-up for new services and set up the data collection tools and regular data monitoring to ensure KPIs are met and/or action plans are developed where they are not being met supported by external partner, Helpforce. Be an active member of the Falls Prevention: Community Exercise Programme Steering Group, providing regular reports back on the progress and impact of the new services Administrative Issue patient data and client workbooks to the volunteers taking on 121 patients in the community/pre-operatively Read and monitor volunteer workbooks (pre-operative assessment / GPs) to measure client progress and escalate issues Add records to Access and CRS (pre-operative assessment patients only) Agree patient record keeping procedures with partner organisations on a needs-only basis Governance Report regular performance and impact data to the FP: CE Steering Group as required Write and end of programme evaluation report for the Kingston Hospital Charity and during the project as requested Input into the development of further funding bids to secure the sustainability of the programme where data indicates it has been successful.
