Description:

The Neurological Conditions Service supports patients with neurological conditions within an integrated neurological specialist MDT, providing co-ordinated rehabilitation and condition management. This can be from point of early diagnosis through to end of life care.
The band 5 post holder will be required to provide holistic and physiotherapeutic assessment of patients with diverse and complex physical, cognitive and psychological neurological presentations in outpatient, domiciliary and community settings. They will be required to develop and deliver individualised treatment programmes as well as provide physiotherapeutic advice and support to other professions within the wider multi-disciplinary team and external agencies.

To provide a holistic assessment to patients with a wide range of neurological conditions. To assess and provide specific physiotherapy intervention as required. You will be required to hold responsibility for own caseload, with supervision from Specialist Neurological Physiotherapists, working within your scope of practice, as an autonomous practitioner. You will be expected to be an effective role model in supporting Band 3 & 4 Rehabilitation Assistants and physiotherapy students.
The post holder will be encouraged to take part in evidence-based audits and service improvement projects to develop own and team’s clinical practice. This includes making recommendations for changes to practice, contributing to the development of the team.

Education and Qualifications
Essential
Physiotherapy degree or equivalent Physiotherapy professional qualification
UK Health and Care Professions Council Registration
Full & current UK driving licence and access to a suitable vehicle.
Desirable
Membership of relevant Specialist Interest Group (e.g. ACPIN)
Membership of Chartered Society of Physiotherapy
Previous Experience
Essential
Experience of working in/ student placements in core areas of physiotherapy including neurology, musculoskeletal, respiratory and rehabilitation.
Experience of working as part of a multi-disciplinary team.
Evidence of time management skills and ability to organise and prioritise own caseload.
Experience of using patient centred goals.
Desirable
Undertaken or participated in a service delivery project, quality improvement project or audit.
Community based experience.
Previous Band 5 experience.
Experience of working within the NHS.
Skills and knowledge
Essential
Clinical knowledge of common neurological conditions.
Experience of being able to adapt communication to match patient needs.
Able to provide teaching/ training to other staff members, patients and their carers.
Experience of applying a variety of outcome measures.
Ability to develop an appropriate treatment plan based on problem lists.
The ability to use the ICF framework to develop and verbalise the clinical reasoning process.
Experience of working with individuals to set patient centred goals.
Awareness of clinical governance and understanding of NHS and challenges facing service delivery.
IT literacy / presentation skills.
Desirable
Knowledge of relevant specialist treatment and adjuncts such as: spasticity management; vestibular rehabilitation, splinting and orthotics, posture and seating systems.
Experience or training in behaviour change models/approaches (e.g. coaching skills, Motivational Interviewing, Bridges Self-Management).
Evidence of ability to learn from past success/ failure
Knowledge of the principles of neurological rehabilitation.
Evidence of flexibility in your assessment and treatment approach.
