To undertake comprehensive holistic assessments for individuals presenting with an acute crisis in their own homes due to deterioration in their health and wellbeing, to reduce or avoid hospital admission To formulate individual treatment plans for patients with complex physical and social needs to ensure immediate patient safety in their own homes To work autonomously in a crisis situation and manage clinical risk associated with this To provide a safe and specialist alternative to conveyance to hospital within two hours of referral To complete onward referrals to specialist teams as appropriate for longer term maintenance and preventio
