The Lead Case Manager will play a critical role in providing wrap-around case management services to pregnant women, single parents, and families, while also supervising and supporting a team of case managers. This position requires strong leadership, organizational, and interpersonal skills to ensure the delivery of high-quality, client-centered services.
The Expertise We’re Looking For
- Associate degree in social work, psychology, or a related field
- Three (3) years of experience in case management, or a focus on maternal and child health, pregnancy support preferred.
- One (1) year of experience in a leadership role.
- Valid Texas Driver’s License

What you Deliver
- Provide comprehensive wrap-around case management services to pregnant women, single parents, and families, including intake assessments, service planning, and ongoing support. Conduct home visits and regular check-ins to assess clients’ needs, monitor progress, and address barriers to success. Coordinate care across multiple systems, including healthcare, social services, education, and employment, to ensure clients receive holistic support.
- Lead a team of case managers, providing guidance, support, and mentorship to ensure effective service delivery and professional growth. Conduct regular staff meetings, individual mentoring sessions, to promote accountability and quality improvement.
- Collaborate with program leadership to develop and implement policies, procedures, and best practices to enhance program effectiveness.
- Provide support to clients facing pregnancy-related challenges, including prenatal care, childbirth, parenting, and family dynamics.
- Collaborate with the Counseling program for group counseling sessions and support groups.
- Assist clients in accessing mental health services, substance abuse treatment, and other specialized supports as needed.
- Coordinate prenatal to postnatal care for pregnant women, including scheduling appointments, arranging transportation, and facilitating communication between healthcare providers. Collaborate with medical professionals, including obstetricians, pediatricians, and nurses, to ensure clients receive comprehensive and timely healthcare services. Assist clients in navigating health insurance coverage, Medicaid enrollment, and other financial assistance programs.
- Connect clients to social and governmental programs, including housing assistance, food support, childcare subsidies, and utility assistance.
- Conduct outreach to community agencies, faith-based organizations, and other service providers to expand referral networks and enhance access to resources. Provide information and support to clients seeking assistance with legal issues, immigration services, and other complex needs.
- Collaborate with program staff for classes and workshops on life skills, personal finance, parenting, and job training to empower clients to achieve self-sufficiency.
- Collaborate with community partners to offer specialized training opportunities, such as resume writing, interview skills, and vocational certification programs. Provide guidance and support to clients in accessing educational opportunities, including GED programs, vocational training, and higher education resources.
