The Outreach Specialist will play a crucial role in engaging with pregnant women, single parents, and families in the community to provide wrap-around case management services and connect them with resources and support. This position requires strong interpersonal, communication, and organizational skills to effectively reach and support clients in diverse settings.

The Expertise We’re Looking For

  • High School Diploma or GED.
  • Two (2) years of experience in outreach, case management, or social services, preferably with a focus on maternal and child health, pregnancy support, or family services.
  • Valid Texas Driver’s License

  • Willingness to travel locally and conduct outreach activities in various settings.

What you Deliver

  • Conduct outreach activities to identify and engage pregnant women, single parents, and families in need of support services. Develop and maintain relationships with community partners, healthcare providers, schools, and other stakeholders to expand referral networks and reach target populations.
  • Utilize various outreach methods, including community events, social media, flyers, and word-of-mouth, to raise awareness of the Pregnancy and Parenting Program and its services.
  • Provide services to clients, including intake assessments and conduct outreach meetings to connect them with appropriate resources and services. Coordinate care across multiple systems, including healthcare, social services, education, and employment, to ensure clients receive comprehensive support.
  • Maintain accurate and up-to-date client records, progress notes, and documentation in accordance with program requirements and confidentiality guidelines.
  • Collect data on outreach activities, client demographics, and service utilization to track program outcomes and effectiveness.
  • Prepare reports, narratives, and other documentation as required by funders, stakeholders, and regulatory agencies.
  • Perform other assigned duties.
