In our quest to deliver patient-focused quality care we are looking to recruit a highly motivated, innovative and enthusiastic Primary Care Clinical Team Lead for 12 months on a fixed term or secondment basis to join our forward thinking, friendly and expanding Health & Justice team at HMP Coldingley. The Clinical Team Leader is a clinical expert who will lead the Primary Care nursing team to achieve a targeted and appropriate service, delivered in accordance with evidence-based practice and local policies and procedures. The post holder is expected to deliver care in a range of nurse led clinics utilising the Quality Outcome Framework and to further develop services such as long-term conditions management, nurse led minor ailment clinics and immunisation programmes.
The post holder will demonstrate a professional manner which is tolerant, courteous and calm in the full course of their duties, in relation to dealing with colleagues, visitors and patients. They will be able to effectively deal with all patients in a professional, nonreactive manner and diffuse potentially volatile situations to avoid escalation.
They will be expected to demonstrate a full and proactive commitment to equal opportunities in accordance with local and national policies.
The post holder will continually promote high standards of service within the Primary Care nursing team, effectively manage poor performance and contribute to the development of a culture of professionalism within the team.
The post holder will work closely with the Primary Care Lead and other Healthcare managers to develop and implement an effective Primary Care Strategy.

Plan, deliver and evaluate interventions for treatment as clinically appropriate using evidence based models
Set up processes/protocols for effective assessment, planning, implementation and evaluation of care for all patients with a range of physical health care needs, including those who may also have mental health and/or substance misuse needs.
Safeguard vulnerable patients in line with Trust safeguarding policies.
Provide brief interventions - health education, advice and information - to patients and develop Health promotion programmes to be delivered in partnership with the prison.
Provide specialist clinical advice, training and support to nursing colleagues on the management of patients.
Apply local Risk Assessment and management protocols.
Make appropriate referrals to other services for long term or complex care management e.g. detox, counselling, psychology.
Advise Primary care nursing staff and GP’s on medication issues, promote adherence to local medication policies/protocols and compliance with legal and professional standards.
Possess knowledge of the security implications of medicines within Health & Justice Services.
Monitor the on-going health status of patients and address identified issues.
Co-ordinate multi-disciplinary clinical practice to provide focused, integrated care and treatment programmes.
Work collaboratively with primary care staff to develop clinical services that deliver high quality nurse-led clinics e.g. minor ailments, chronic disease clinics and health promotion initiatives.
Use highly skilled interventions to de-escalate aggressive patient behaviours and be aware of the appropriate processes for dealing with
Work collaboratively with staff of other disciplines e.g. prison, education staff to provide integrated and coordinated care.
Work within own sphere of competency and ensure that training needs are communicated to the team manager.
Lead local audits and participate in trust audits to enhance quality of care
