Primary Care HCA (Integrated Services)

To work in line with Haringey GP Federations organisational policies and procedures. To deliver clinics according to your clinical competency, general practice, and service needs. To conduct physical health checks and appropriately escalate queries to relevant Nurse supervisor, GP or Clinical Lead. To promote a culture of shared decision making to support patients through decision making processes.
To promote a seamless service of care for patients through liaison with all members of the healthcare team within primary and secondary care. Work in accordance with internal administrative systems relating to but not limited to the management of clinical data, in line with GDPR. To promote and provide evidence-based care that complies with the relevant legal and policy frameworks. To work collaboratively with multidisciplinary teams across a range of care settings.
To always respect patient confidentiality and not divulge patient information unless sanctioned by the requirements of the role
