We are looking for an experienced Learning disability nurse to join our Primary care liaison Team in the City of Nottingham.

Come and join a forward thinking and innovative team to help promote the needs of individuals who have an Intellectual disability and support them and services to enable them get the physical health care they need.
Be part of a team that breakdowns barriers that people with Intellectual Disability experience when accessing primary health care services and support Intellectual disability patients to access National Screening programmes.
Have an active part in delivering enhanced service training to primary care colleagues and supporting the GP Practices in keeping their Learning Disability Registers up to date.
The post is a permanent part - time position.
22.5 hours a week.
This is an really exciting opportunity that has arisen within the Team and we are looking for a passionate individual to join us and make a real difference!
This post is eligible for a Golden Hello payment of £2,750 for new starters who are external to the Trust. This is payable in 3 instalments over a 2 year period. This payment is only eligible to new starters who do not already hold a contract of employment with the Trust. This post also includes paid enhancements for unsocial hours working during night, weekends and bank holidays.
Liaise with Primary Care, IDD Community Services, Patients, carers and family to help facilitate and promote reasonable adjustments required for people with a Learning Disability to access Primary Health Care.
To educate primary care professionals regarding the Health Needs for people with Learning Disability
To work alongside people with a learning disability so that they are able to access and understand primary care they require.
Support patients to access mainstream screening services.

We are looking for a RNLD nurse who has experience of working with people with intellectual disabilities in a health or social care setting.
You will be part of the Primary Care Liaison Team working within Nottingham City.
Your key role is to enable GP Practices by supporting and liaising with practices to keep their learning disability registers updated annually across the city of Nottingham.
You will support the Team around the development of health screening and promoting reasonable adjustments for people with intellectual disabilities in the City of Nottingham.
You will support primary care colleagues by promoting and advocating the health needs of those with intellectual disabilities within Nottingham city.
you will enable patients with intellectual disabilities and their family’s to navigate their patient journey through primary care services
