Description:

The Digital & Transformation Manager will support the leadership of the Four Counties PCN but also work across all Lakeside Healthcare led PCNs to support with specific projects appropriate to their skills and organisational requirements. Working with clinical and operational teams, including Clinical Directors, the role holder will ensure the PCN contract is delivered to standard, deliver service improvement and efficiency, identify opportunities to make tangible positive transformation and execute change in a collaborative way. The role holder will be key to the vision of the Four Counties PCN but also contribute to the vision for all Lakeside Healthcare led PCNs. Working collaboratively the post holder will support the delivery of strategy related to transformation and improvement.
Supporting the Clinical Director they will lead the implementation, embedding and evaluation of practical improvements that make a difference to patients and staff, optimising the use of technology, ensuring they address health inequalities and championing diversity, inclusion, and sustainability of general practice. This will be achieved through optimising digital tools, effective use of data, operational processes, pathways, new ways of working and embedding transformation and change. The post holder will also support PCN colleagues in identifying costs and financial implications of transformation initiatives, including the use of digital tools. The post holder will work closely with staff and management working at practice and PCN level, to understand current service models, identify challenges, agree priorities for improvement and then support these to be delivered.
The post holder will develop strong relationships with all PCN member practices to support the delivery of improvements that will provide benefits both at individual practice level and at the level of the PCN. KEY PERFORMANCE AREAS Operational Ensure all relevant national strategies, policies, targets, and frameworks are fully implemented and delivering in line with expectations. Provide accountability and oversight to the delivery of PCN contracts and specifications. Working with the Strategic Digital and Transformational Manager explore potential new digital and transformative projects.
Drive issues through to resolution, acting as point of operational expertise. Understand the type and intensity of support/training needs of the PCNs and coordinate this support, including through OD programmes, working closely with the ICB Primary Care Training Hubs Where required mentor and manage staff across the PCNs, helping them to develop their skills; proactively assess competency levels and design support, guidance, and advice to ensure that efficient service and quality standards are maintained. Working with key stakeholders develop a coherent workforce plan to support recruitment. Provide leadership and manage teams to achieve goals.
The role will include line management either on a permanent or temporary basis. Support the PCN estate projects and wider strategies. Business and Service Improvements / Quality Improvement Working with key stakeholders support the design and delivery of a strategic vision and identify supporting objectives. Accountable for operationalising strategy and services the post holder will support the PCN boards with high-quality hands-on operational knowledge, leadership, and support.
Work with the Strategic Digital and Transformation Manager, using agreed project management methodologies and approach, to manage the effective implementation of change. Ensure objectives and priorities for network delivery are in line with operational capacity, financial parameters, and contract requirements, making sure physical and financial resources are managed and maintained for maximum effectiveness. Work with PCN practices to ensure new care models are implemented and key processes and standing operating procedures developed, to support the integrated care requirements. Lead a multi-disciplinary improvement approach to change, including building capability for quality improvement and system wide approaches to problem solving.
Facilitate clinically led innovation and the effective adoption of improvement initiatives, including integrated working at neighbourhood and/or place level, to improve access to services for patients and the overall patient experience. Information and Data Management Improve adoption and/or optimisation of new technology to enhance patient access and experience, population health management and increase PCN and/or practice productivity. Take a data driven approach, looking at evidence and information available to support understanding of challenges, identify possible solutions and track the impacts of changes that have been made. Improve overall data quality and extraction to understand demand, capacity, and activity.
Using this to drive improvements in patient access, experience and operational efficiency, better matching of capacity to need and staff experience at work. Effectively present and communicate data to practices and PCN staff at all levels to increase understanding of operational challenges, highlighting variation and supporting open discussion. Analyse and assess performance and capacity, working with stakeholders to identify areas for improvement, supporting member sites and operational teams with agreed changes to services. Support the Strategic Digital and Transformation Manager with the development of PCN dashboards, that provide monthly performance analysis of KPIs, across a range of areas e.g., performance in the DES, QoF, IIF, Enhanced Access etc.
Review current technologies to identify and implement business efficiencies e.g., increased uptake for patients requiring vaccinations/health checks etc with reduced manual intervention. Communication and Engagement Foster a culture of collaboration and promote learning to drive continuous improvement. Take an active role in the development and embedding of the Lakeside Healthcare PCNs culture, values, and reputation. Facilitate actions to promote staff satisfaction and retention.
Work flexibly to support the wider teams when necessary. Ensure effective communication amongst all stakeholders including patients. Promote engagement internally and externally, always acting as an ambassador. Proactively build relationships with practices to help influence proactive and new models of care.
Participate in relevant working groups, project groups, service meetings and other forums across the PCNs to agree changes, share learning, discuss challenges, and resolves issues. Participate in development of effective systems of communication and co-operation between member practices, staff, and other members of the primary care team. Represent the PCNs in external groups or with external bodies as required, including in potentially sensitive situations. Policy and Service development With responsibility for supporting the PCN to make progress with transformational change, the post holder will have expertise in understanding and interpreting national and system-wide policy developments and initiatives to meet local circumstances and contexts.
Maintain a good knowledge of emerging policies and/or directives from the Department of Health and Social Care, NHS England and Improvement and other relevant government departments and agencies. Supporting understanding and interpretation locally. Work with the Strategic Digital and Transformation Manager, and ICB as required, to support the development of overall primary care transformation strategies and their delivery. Support the PCN and member practices to adapt and apply specific policies and initiatives to local needs and to prioritise national or system level initiatives based on local requirements.
Compliance, Information Management and Reporting: Accountable for the capture, maintain and baseline and performance data and information, as part of supporting a PCN reporting cycle and performance management requirements. To be linked to supporting and tracking of service improvement and the realisation of key patient and service improvement benefits. Support functions covering planning, tracking, reporting, quality management and internal communication. Ensure that Lakeside Healthcare corporate systems and processes are adopted and embedded in new ways of working where appropriate (Radar and Rotamaster)
