Description:

The Senior Primary Care Manager will lead on the transformation and delivery improvement of primary care across West and Central Lancashire sub-place. They will lead a team working with all primary care providers in the continuous improvement of the services provided.
The role will be varied and will adapt over time as the ICB develops.
However, the post holder is expected to fulfil the following key functions: -
Provide overall management for the placing team whose remit includes the transformation and delivery improvement of primary care providers
Working within the overall strategic objectives to devise, implement and monitor strategy for transformation and improvement of primary care providers across Central and West Lancashire
Develop effective working relationships across the LSCICB including Place Directors and Local Authorities
Develop effective working relationships within the PACC team including close links with the Delivery and Delivery Assurance Teams
Develop effective working relationship with ICB functions, including, but not limited to population health, quality and finance
Interpret and implement complex national guidance, applying local and national policies to meet the needs of a range of local populations
To provide expert advice on primary care within place on strategic developments
To work with the Head of Delivery Assurance to ensure the contractual compliance and delivery improvement of all primary care contractors across place
To manage the budgetary implications of primary care provision within the place team
Manage complex relationships and competing demands across the place team
The post holder will have regular contact with a wide range of staff at all levels within the ICB, with senior and other representatives of local stakeholders, as well as with patients and their representatives. Some of the key direct relationships include:
Lancashire and South Cumbria ICB executive teams
Lancashire and South Cumbria ICB leadership teams
Chief Information Officers / Digital Leads
Clinical Leaders
Colleagues within NHSE/I
Senior leaders across Lancashire and South Cumbria, NHS, local authorities and other third sector organisations
Leading transformation
The post holder will take the strategic leadership of a range of contracts/ portfolio areas and be responsible for the transformation, delivery and programme management of these areas in order to achieve the triple aim of improving patient experience, quality and financial outcomes.
Working with the Associate Director for Primary Care
Secure quality, safe, robust and appropriate commissioning of services, which provide value for money for the local population and promote integration.
Ensure delivery of the primary and integrated neighbourhood care teams annual operating plans and support delivery of designated wider ICB projects.
Leading delivery improvement
Lead a portfolio of place based and clinically led commissioning including service transformation, redesign and reconfiguration using continuous improvement, change process and effective programme management.
Provide clear direction for the areas of responsibility that is in accordance with the organisation’s values and the ICBs constitution
Provide dynamic leadership and guidance to senior management and primary care providers, creating a compelling vision for the future and communicating this within and across organisations
Working with senior colleagues across place to create a robust and sustainable organisational structure with the capacity and capability to meet statutory obligations of the ICB in terms of quality and effective use of public funds
Support the development of, and embed collaborative and effective partnership working across health and social care providers, commissioners, Local Authorities and other NHS and non-NHS organisations and stakeholders
Ensure that the organisation values diversity and promotes equality and inclusivity in all aspects of its business
Work with clinical colleagues, the public, patients and partners to establish and communicate effective strategies for the ongoing improvement of health services
Be visible and accessible to primary care providers and partner organisations
The post holder will be an active member of local/regional networks where appropriate and agreed and ensure that processes are in place to facilitate effective collaboration and communication.
Clinical Engagement
Work with the ICB Clinical Leads to develop a culture that ensures a strong clinical voice and actively engages local primary care clinicians in shaping the direction and goals of primary care
Ensure that the interests of patients and the community remain at the heart of discussions and decisions
Strategic Commissioning & Transforming Clinical Services
Challenge current practice, including clinical practice and develop new ways of working that improve the quality and effectiveness of services
Ensure the ICB is responsive to the views of local people and promotes self-care and shared decision-making
Work collaboratively across organisational boundaries to secure improved outcomes for patients, local communities and the wider population and make necessary arrangements to ensure the effective commissioning of services
Create pathways of care across primary, secondary and tertiary care that deliver improvements in healthcare outcomes and efficiency
Ensure robust performance management of commissioned services against agreed performance targets, taking remedial action where necessary
Understands commissioning risk environment including understanding of the strategies that have been adopted by the ICB to manage and mitigate these risks
Ensure patients and the wider public are actively involved in the design of healthcare services and that this feedback is used to enhance choice, quality and efficient service provision
Contracting finance and quality
To lead with the Head of Delivery Assurance on performance management of contracts within portfolio area
Budget holder accountability for the portfolio
Support the ICB in the delivery of a balanced budget and ensure, with financial support, full compliance with financial regulations
Support the ICB in ensuring that the regularity and propriety of expenditure is discharged, that arrangements are put in place to ensure good practice is embodied and funds are safeguarded through effective financial & management systems.
Work with the ICB to ensure they makes decisions which secure the best use of public money
Policy creation/edit/updates
Lead the development of integrated neighbourhood care strategies, operational policies within the area of responsibility
Quality
The post holder will ensure that there are systems in place to support inter- organisational quality assurance and Quality improvement.
Research and development
The post holder will ensure they remain up to speed on all the latest national and local developments, best practice, emerging evidence to support effective commissioning
Autonomy/level of decision making
The post holder will have full autonomy within the directorate they are responsible for, within the limits set out by the ICB-wide processes and controls and as agreed with the ICB executive.
The main duties and responsibilities described above are not exhaustive and the post holder maybe expected to take on other responsibilities or specific tasks as required as requested.
Further, over time, it is likely the remit and requirements of the role will evolve, and the post holder will be required to adjust their working approach and style to accommodate this with the full support of line managers
