Description:

The role of PCN Lead Practitioner is fundamental to the developing of these innovative new teams and mental health services based within networks of GP Practices (PCN’s). To provide a sustainable, person-centred system of community mental health care, which delivers better access to a broader range of care options, reduced referral to specialist services, improved patient experience and outcomes. It will aim to include evidence- informed approaches that are known to impact on both mental and physical health.
The role will support the ongoing development and mobilisation of integrated primary care mental health services. The role provides and promotes early assessment, treatment and/or onward bridging to community resources and support and improved access to evidence-based interventions for people with serious mental illness. The role will help people to focus on achievable goals and access local community resources. The service will be for people over 18 years (including carers) with a specific focus on:
The post holder will be responsible for providing clinical leadership and the operational management of staff to a newly formed Primary Care Mental Health Team.
They will use effective leadership, change management and team building skills to monitor, evaluate and develop the service provided. The practitioner will provide a “bridge” between the primary care mental health team and secondary care.
In keeping with the national guidance on the introduction of this role, it is envisaged that the focus of a practitioner’s work will generally be people whose needs cannot be met by Talking Therapies services but who may need not need ongoing care from secondary community mental health services.
They will work alongside general practice teams, providing clinical assistance and advice to the GPs and other members of the primary care team in managing the mental wellbeing of the practice population. Learning from National Early Implementer sites, there will be a requirement to build relationships with other community providers and mental health services while developing our PCN hub and spoke model.
It is envisaged that the role will be instrumental in developing a Community Mental Health MDT which will include the lead GP, consultant psychiatrist, Talking Therapies representative, homeless health service, substance misuse service and secondary care assessment nurse.
