This program is devoted to clinical effectiveness, efficiency, and analytics through the creation and maintenance of comprehensive clinical pathways. This position will facilitate multidisciplinary teams to develop clinical pathways, standardize the work product across teams, and ensure appropriate integration. Also collaborate with clinical analytic colleagues and clinical decision support teams to to evaluate, define, and establish goals for pathway utilization and effectiveness.

Job Description

  • Support the clinical pathway program in the implementation, coordination, and monitoring of clinical pathway development, program priorities and barriers, and maintenance of a pathway database for internal and external use. Regularly monitor pathway teams progress and facilitate resolution of barriers to progress in a standardized fashion across all pathway teams. Provide routine reporting on program progress to appropriate leaders and oversight bodies. Track program priorities and work with leadership within Office of Quality and Patient Safety to assign resources to facilitate the approved timelines and directives of the Clinical Pathway Steering Committee. Facilitate the creation and maintenance of an internet site that can be made available to external customers.
  • Organize and coordinate regular reviews of pathways to ensure new empirical evidence is integrated and dispersed across clinical care. Establish ongoing monitoring strategies and processes with each pathway team to ensure all pathways and empirical evidence are reviewed at regular intervals, references are maintained, and pathway owners and sponsors are up-to-date. Ensure new empirical evidence is documented and made available to pathway database users. Establish on-going performance and utilization expectations, and routinely investigate utilization data to identify opportunities for improved adoption.
  • Facilitate the work of multidisciplinary teams in the development of comprehensive clinical pathways: not limited to initial discovery discussions with project SMEs, charter development and scoping decisions, proposal and prioritization, team kick-off meetings, and subsequent working sessions. Attends pathway team meetings and ensures that pathway projects are following a standard template for development and outcome. Ensure institutional procedures are followed by pathway teams. Ensures pathway progress and documentation is maintained and standardized across pathway development teams. Responsible for monitoring and updating timelines for pathway completion and communicating appropriately to project management, operational, and technical resources.
  • Collaborates with Information Services Department and Clinical Analytic colleagues to identify and adopt new clinical decision support tools to improve clinical effectiveness monitoring and adoption. Collaborates with staff to identify opportunities for data automation, further increasing efficiency in reporting. Work with clinical analytic colleagues to evaluate, define, and establish goals for pathway utilization and effectiveness, and routinely monitor for all pathways at appropriate intervals.

MINIMUM EDUCATION AND/OR TRAINING:

  • Bachelor’s degree in a related field.
  • Masters, PhD, or other advanced degree preferred.

MINIMUM EXPERIENCE:

  • Five (5) years’ experience in hospital-based quality/safety activities or any combination of directly related education and experience required.
  • Two (2) years project leadership and management experience required.
  • RN experience or clinical background preferred
  • EPIC and Agile MD experience preferred
