Description:

Airedale Hospital are working collaboratively with Yorkshire Cancer Research and Sheffield Hallam University to implement the Active Together model for people diagnosed with cancer.
The Active Together programme will run alongside the existing diagnostic and treatment pathways to ensure patients are optimally prepared for treatment and beyond. It aims to maximise resilience to treatment, improve longer term health, minimise side effects and empower patients to be partners in their healthcare.
The programme combines physical activity, nutritional support and psychological support alongside the clinical pathway.
This is a funded 5 year programme which will have staged implementation into all relevant cancer pathways. Patients will receive individual assessments and be triaged into a support pathway suitable to their needs. This may be delivered within locality or selected facilities closest to the patient’s home or may be a self-management pathway.
As Programme Manager with previous NHS project management experience you will be responsible for developing and delivering the service for cancer patients in partnership with key stakeholders within the locality.
The post holder will be responsible for delivering the program which will improve treatment pathways, patient outcomes and experience. You will provide hands on leadership using Lean based improvement methodology to deliver locally determined quality improvement activities within the scope and direction of the Active Together Programme and Cancer Alliance Work programme.
Facilitate and lead the implementation of the programme by managing and supporting teams to embed and maintain the service.
Lead and deliver the project as above including presenting analysis of reports and embedding processes.
Evaluate roles and responsibilities to identify change opportunities to deliver optimal care and outcomes.
Organise and facilitate key events to embed a cost efficient and effective pathway delivery.
Champion, inspire and lead the development of a culture where improvement is a core component of every member of staff’s daily routine
Promote outstanding care and work in a way that is patient-centred, involves service users, responds positively to feedback and promotes learning and continuous improvement
