MINIMUM JOB REQUIREMENTS:

  • A recognised third level qualification to degree level in a relevant discipline such as Psychology, Social Work, Youth Work or Addiction Studies.
  • Key working skills.
  • Knowledge of harm reduction initiatives and presenting needs of rough sleepers.
  • Knowledge of Case and Care Management Protocols and systems
  • Three years experience working in a Social Care setting.
  • Managing a caseload and managing time effectively.
  • Experience of building up contacts and good working relationships with a variety of stakeholders appropriate to the job.

PREFERRED JOB REQUIREMENTS:

  • Qualification in Key working/ Case Management
  • Driving licence.
  • Knowledge of rights and entitlements of homeless persons.
  • Knowledge of motivational techniques and tools to assess readiness to change.
  • Practical experience of assessment, planned support, key working and advocacy.
  • Experience working with people in active drug addiction using a harm reduction approach.

Responsibilities:

  • The project worker will work closely with the Outreach Service Manager and the team and take the lead on key projects and complex cases, specifically to:
  • Implement the Care and Case Management processes and protocols.
  • Deliver a quality effective service ensuring the progression and sustainability of all clients.
  • Case Manage 20 clients, develop and implement their support plans, and work as part of a multi disciplinary team to achieve service and clients targets.
  • Engage clients with appropriate accommodation and support services required as identified in support plan.
  • Deliver the Emergency Needle Exchange Programme and other street based Harm Reduction interventions.
  • Utilise PASS and other data recording systems.
  • Carry out assessments with people sleeping rough and provide follow up support to facilitate access to accommodation (short and long term), residential treatment and medical services.
  • Provide in-reach and out-reach to people sleeping rough and homeless people at risk of rough sleeping.
  • Induct new team members and carry out relevant on- the-job training and guidance.
  • Utilise evidence-based techniques such as motivational interviewing to explore clients’ motivation to change.
  • Respond and participate in media and fundraising requests regarding the service and service provision.
  • Perform all the administrative duties relating to the role in order to ensure that clients are supported in an effective and timely manner.
  • Work in cooperation and partnership with other agencies, both voluntary and statutory ensuring good communication and cross-agency flexible working practices to minimise obstacles to a person getting access to supports, services and/or accommodation.
  • Identify and manage risks and ensure the protection of all clients.
  • Engage with and build a professional trusting relationship with clients.
  • Responsible for the achievement of goals as determined by organisational strategy and local scorecard.
  • Ensure clients are consulted and involved in the operation of the project as far as possible.
  • Build resources and strong community relationships to improve the continuity of care.
