Description:

The post holder will: Contribute to the realisation the outcomes for North East Lincolnshire, in particular to enable all people in North East Lincolnshire to feel safe and be safe, to have good health and wellbeing and to realise their potential through skills and learning. This will be achieved through the effective delivery of a range of support to young people and families in the locality, in order to improve their health and wellbeing and reduce the need for statutory intervention. Main Responsibilities Professional/Clinical Autonomous practitioner who will: 1. The post holder will work with Specialist Childrens Public Health Nurse on a caseload and will agree work to be undertaken with the Specialist Childrens Public Health Nurse within the team.
2. Take an active part in the expanding public health work undertaken by the Specialist Community Public Health Nurse by undertaking health assessments and reporting any concerns or problems back to the Practitioner, health education and health promotion as decided by the senior staff. 3. Manage a caseload of children aged 0-5 years (include children with special educational needs and disabilities), taking the lead from public health colleagues where need is identified.
4. Identified need or concerns will be raised with the appropriate Specialist Community Public Health Nurse dependent on the age of the child. 5. Understand typical ages and stages of child development including the development of children disabilities and additional needs.
6. Maintain clinical records either in paper or electronic form in accordance with North East Lincolnshire Council and Nursing and Midwifery Council (NMC) policy. 7. Be able to work unsupervised but work will be designated, monitored and reviewed by the team manager and/or a Specialist Community Public Health Nurse within the team.
8. Work with colleagues to plan and deliver health promotion and health education, as part of a programme of health interventions connected to public health priorities. 9. Assess and analyse the needs of individual children and young people, develop and implement an appropriate package of care.
This may include joint work with parents/carers and schools to develop an appropriate care plan 10.Make appropriate referrals to colleagues and partner agencies, following discussions with the Specialist Community Public Health Nurse within the team. 11. Participate with supervision and clinical audit as part of the health team. 12.
Report to Childrens Public Health Team manager when demands of work load are not being met because of sickness, holidays or lack of resources. 13. To contribute to deliver support, advice and signposting to families, professionals and other agencies via the school nurse duty line. 14.
Undertake such other duties as may be reasonably expected at this level. Additional Responsibilities Communication/Relationships 1. Develop strong communication links, both written and verbal, between members of the team, education staff, children and young people, parents and carers, other professionals and agencies considering the need to ensure that all information is relayed in a manner appropriate to age, culture, language and understanding. 2.
Demonstrate an ability to convey information to other team members, parents, children and young people and other professionals and agencies using tact and empathy. 3. Attend professional development group meetings passing on to the rest of the team any work they are involved in to update them 4. Notify senior staff of resources required and when routine stock is running low.
Documentation 1 Ensure that all personally written records and activity data are maintained in accordance with professional and Council standards. Leadership and Line Management 1. Review and reflect on own practice in line with NMC and local guidelines by means of annual appraisal, clinical supervision, and personal reflection. 2.
Ensure preparation for timely revalidation on the NMC register. 3. The post holder will act as a mentor to student nurses and new health screening practitioners to the team and as a preceptor to any new Public Health Nurses joining the team once relevant training has been undertaken. Professional development 1.
Identify through annual appraisal where training needs lie and undertake appropriate training as defined through this process. 2. Maintain own portfolio in line with NMC requirements. 3.
Undertake mandatory training and clinical updates as required by the Local Authority. Training & Development 1. Act as a mentor for students and preceptor for new staff to the team once trained and competent. 2.
Act as a resource/educator for age 0-5 provision. Service development and delivery 1. Participate in the operational planning and implementation of policy and service development within the team and commenting on Council policies. 2.
Ensure stock levels are maintained for health promotion and health education and advise on resources required. Clinical Governance and quality standards Contribute to the Councils clinical governance arrangements and quality agenda, including monitoring of practice standards. Research and Practice development Undertake literature searches as appropriate for practice and audit as part of the team and feedback results as required by the team. General 1.The current main organisational priorities for the post.
In the context of rapid change taking place within the Council, these priorities will develop and change in consultation with the post- holder in line with service business needs and priorities. Other work and general duties as delegated within the overall purview of the post. 2.
