Description:

We are seeking to appoint a Band 6 Pulmonary Hypertension Clinical Nurse Specialist following the promotion of our current post holder.
The pulmonary hypertension service at Hammersmith Hospital provides diagnostic investigations, outpatient and inpatient treatments along with long term follow up for people with pulmonary hypertension.
The MDT strive to improve the quality of life and personal experience of patients with Pulmonary Hypertension by utilising the best evidence based treatments and care - the Nurse Specialists are a key part of the multi-disciplinary team
The successful candidate will be required to support the team in training patients to administer specialised drugs, supporting patients and their relatives with the impact of Pulmonary Hypertension, supporting clinical trials and recruiting patients into research studies as well as providing training for staff.
You should be able to demonstrate excellent communication skills as it is an essential part of the role - direct communication with the medical team, patients relatives and other members of the multi disciplinary team who are actively involved in the patients treatment plans.
The Directorate will offer further development that can be tailored to the successful candidate/s and include both leadership and clinical components.
Undertake full nursing assessment, implementation and evaluation of patient, and families/carers, to provide specific expert nursing care, advice, support and counselling where necessary.
Proactive participation in the co-ordination of patient care throughout the patient pathway, developing the pathway across the various settings to maximise patient satisfaction and outcomes.
Undertake physical examination, assessment and discharge of routine daycase patients, and dictate informative discharge summaries for local primary and secondary care physicians.
Attend multidisciplinary team meetings, and advise its members and those of the wider health care community, to enable the provision of high quality patient care. This might include advice on physical care, symptom control; care planning, social and psychological care, communication, resources and equipment.
Act as patient’s advocate and counsel when informed discussion may lead to choices being made concerning treatment options or quality of life issues. Ensure that all relevant information is accurately documented in the patient health record.
Register patients with homecare providers to ensure regular supplies of disease targeted therapies, pumps and consumables.
Monitor patients progress, work towards safe and timely discharge plans and ensure barriers to discharge are identified and acted on appropriately.
