Description:

Accept referrals via agreed protocols within the service.Provide screening, assessment and interventions for young people presenting with common mental health problems with a goal of self-management and recovery. Undertake patient-centred interviews which identify areas where the young person and their parents/carers wish to see change and/or recovery and make an accurate assessment of risk to self and others. Make decisions on suitability of new referrals, adhering to referral protocols, organising assessment, signposting to appropriate services and facilitating referral to specialist services where required.
Provide a range of information and support for evidence based high volume low intensity psychological treatments. This will include group work; guided self-help, evidence based parent led intervention. This work may be face to face, telephone or via other media. Educate and involve family members and others in treatment as necessary.Adhere to an agreed activity contract relating to the number of client contacts offered, and clinical sessions carried out per week in order to minimise waiting times and ensure treatment delivery remains accessible and convenient.
Attend multi-disciplinary meetings relating to referrals or clients in treatment, where appropriate. Complete all requirements relating to data collection within the service. Keep coherent records of all clinical activity in line with service protocols.Work closely with other members of the team ensuring appropriate step-up and stepdown arrangements are in place to maintain a stepped care approach. Assess and integrate issues surrounding education into the overall therapy process.
Ensure the maintenance of standards of practice according to the employer and any regulating bodies and keep up to date on new recommendations/guidelines set by the department of health (e.g. NHS plan, National Service Framework, National Institute for Clinical Excellence). Be aware of, and keep up-to-date with advances in the treatment for common mental health problems. Ensure clear objectives are identified, discussed and reviewed with senior therapists on a regular basis as part of continuing professional development.
Keep up to date all records in relation to Continuing Professional Development and ensure personal development plan maintains up to date specialist knowledge of latest theoretical and service delivery models/developments. Attend relevant conferences/workshops in line with identified professional objectives. Attend clinical/managerial supervision on a regular basis as agreed withManager/Clinical lead. Ensure that client confidentiality is protected at all times.
Maintain confidentiality of information at all times in line with the requirements of the Data Protection Act and ensure that client confidentiality is protected at all times. Be conversant and comply with the Trusts Health and Safety Policy and report as necessary any untoward incident or hazardous event utilising the trusts Datix System.
