Description:

An exciting opporunity has arisen for an Enhanced Clinical Practitiner (ECP) to work as part of a small team of Community General Practitioners and ACPs supporting multi-disciplinary teams for North Hardwick and Bolsover PCN.
You will work with patients and their carers and other clinicians, either in person or remotely, supporting clinical practice and the development of the Team Up Service.
Working across primary, secondary, and social care services including housing, the voluntary and independent sector, you will be required to assist with and perform the provision of holistic, high quality, evidenced based health care.

Castle Street Medical Centre - Bolsover, Crags Healthcare - Creswell & Whitwell, Creswell & Langwith Surgeries, Emmett Carr Surgery - Eckington & Renishaw, Family Friendly Surgery - Bolsover, Shires Healthcare (Shirebrook and Church Warsop, Welbeck Road Health Centre (Bolsover)
This role will be hosted by DCHS , the Team UP Home visiting Service work on behalf of the PCN.
To provide expert Enhanced clinical management for patients who are frail and/or with multiple long-term conditions who are at risk of deteriorating health that may result in declining quality of life or avoidable hospital admission and unnecessary lengths of hospital stays.
To provide Enhanced level clinical assessment, determine diagnosis, initiate appropriate clinical treatment, refer appropriately and lead to co-ordination of care, working with partner agencies and key professionals.
To provide expert clinical advice to patients’ and other professionals involved in their care (according to the dependency of patients) needing complex Chronic Disease Management/Case Management.
Working to a defined local population, within an appropriate supervisory and monitoring framework, to an agreed, multi-disciplinary treatment plan, providing ongoing treatment, teaching and instruction to referred patient caseloads.
