QUALIFICATIONS

• Successful completion of a Bachelor’s Degree in Health Administration, Business or Science (Health Profession preferred)
• Canadian Patient Safety Program Certificate or previous Canadian Patient Safety Officer Certificate (or to be obtained within a year)
• Minimum five (5) years recent, related job experience
• Knowledge of governmental and other regulatory standards, requirements, guidelines and industry best practices related to; Patient Safety, Quality Improvement and Accreditation
• Knowledge and experience with root cause analysis, cause and effect analysis, and failure mode and effects analyst (FMEA) as applied in healthcare.
• Advanced verbal and written communication skills.
• Ability to apply a wide range of qualitative and quantitative analyses
• Sound judgement and initiative for priority setting and strong project management skills
• Demonstrated ability to establish effective interpersonal relationships
• Advanced knowledge of computers and software (ie. MS Office Suite)
• Demonstrated practice and commitment to the principles of patient and family centered care
• Demonstrated practice and commitment to patient and staff safety at LHSC
• Demonstrated practice and commitment to LHSC’s vision, purpose and values
• Demonstrated ability to attend work on a regular basis

IMMUNIZATION REQUIREMENTS:

Before beginning employment at LHSC, all new hires will be required to:

• Provide vaccination records or proof of immunity against measles, mumps, rubella and varicella (chicken pox), Hepatitis B, Tetanus/ Diphtheria/Polio; Meningitis;
• Provide documentation of the Tuberculosis skin testing (two step);
• Provide proof of Health Canada-approved COVID-19 vaccination reflective of Ontario Health’s current definition of being fully vaccinated
