Hours: 37.5 hours per week
Salary: $40.61 - $50.76 per hour (To commensurate with experience and consistent with UHN compensation policy)
Shifts: 9.00 - 5.00pm, Week days
Status: Temporary Full-Time (approx. 18 months until October 2025)

POSITION SUMMARY
Toronto Western Family Health Team (TW FHT)’s program links with the surrounding community to provide primary care to families and community members, with special attention to the needs of the aging population, children and women and patient populations who include: a) underserved, at-risk and high needs patients, c) adolescents and emerging adults and d) patients with mental health and addictions.
With the teamwork of physicians, nurse practitioners, primary care nurses, social workers, pharmacists, physiotherapists, occupational therapists, respiratory therapists and chiropodists.
The Quality & Project Lead plays an integral role in assisting the Executive Management team of the Toronto Western Family Health Team (TW FHT) to identify, develop, implement and monitor strategic and quality improvement initiatives. The Quality & Project Lead will seek out and support a broad range of partnership opportunities that help build and sustain operations while aligning with the strategic goals and objectives.
QUALIFICATIONS
- Completion of a Bachelor’s Degree program in a registered health profession or Computer Science, Computer Engineering, Health Informatics or recognized equivalent.
- Master’s Degree, preferred
- Formal Quality Improvement education and training (Traditional or clinical)
- At minimum, 4 years of practical and related experience
- Excellent analytical, process analysis, problem recognition, problem solving, interpersonal, verbal and written communication skills
- Quality Improvement experience and expertise in healthcare environments
- Demonstrated end-to-end project management and change management experience
- Must be able to work independently and prioritize workload to meet deadlines.
- Excellent relationship management
Responsibilities:
- Leading, planning and developing processes to achieve desired outcomes for all aspects of assigned projects
- Defining / developing methodology to achieve results
- Supporting the development and implementation and monitoring of program strategic plans
- Developing proposals, as required
- Leading the implementation of key projects where appropriate
