How you make a difference:
The Re-entry Specialist works to reduce recidivism for individuals completing the re-entry program.

Key Responsibilities:

  • Serve as a community agency point of contact, coordinating mental health center referrals and follow-up appointments, ensuring seamless access to services.
  • Assess and coordinate benefits needs, collaborating with jail-based services to ensure timely submission of benefits applications.
  • Evaluate client access to prescribed psychotropic medications upon return to the community, assisting with Physician’s Assistance Program applications and facilitating communication with treating psychiatrists for medication adjustments if necessary.
  • Assess client housing access and service requirements, coordinating referrals to address housing needs.
  • Ensure compliance with legal obligations post-discharge, including documentation of court dates, registration, and follow-up with court-ordered community services, while also maintaining continuity of care by conducting post-discharge follow-ups with both the jail of origin and the discharged patient, when feasible.

Qualifications & Requirements:

Education

  • Bachelor’s degree in social work or counseling; Master’s degree is preferred.

Experience

  • 2 years of experience with the severe and persistent mentally ill is preferred.

Licenses/Certifications

  • None required.
