Description:
1.To provide supervision and support to the staff team to implement care plans and treatment programmes in conjunction with the relevant teams, record their progress and provide recommendations within the review process. 2.Support the management team with day to day operational duties. 3.Work across partnerships/systems as part of an integrated team. 4.To act as shift leader and take responsibility for identified Unit/s.
5. To be responsible for ordering, checking, storing and administering medication in line with appropriate policies and legislation. 6. To formulate risk assessments, care plans and other key documentation, with service users and key professionals to ensure a person centred approach to care practices.
7. To participate in the recruitment, absence management and where necessary the performance management of care staff in accordance with existing policies and procedures. 8. To identify any deterioration in individual service users and ensure that support from the relevant professional is sought in a timely manner and that all key documentation is completed.
9. Communicate effectively with other team member, people who use our services, relatives, carers and other agencies involved in the support and care of the service user. 10. To ensure all IT systems are kept up to date.
11. Support staff to ensure service users are encouraged to maintain or achieve independence and self-determination. Whilst this may entail acceptable risk taking, due regard must be given to service users safety. 12.
Participation in the planning of person centered care programmes and the preparation of case reviews in partnership with the wider care team. 13. Involvement in and promotion of the emotional and psychological needs of service users to instil confidence by encouraging feelings of security and personal worth. 14.
Guide and support staff to assist service users to participate in domestic tasks (e.g. cooking, laundry, care of linen and clothes, washing up and other kitchen duties, cleaning and bed making) with a view to regaining independence. 15. Assist with the control of infection, including the control of clinical waste in line with existing policies and procedures 16.
Actively lead the team to promote a consistent approach to meet the Units objectives. 17. To have regard at all times to the comfort, well-being and self-respect of all service users. 18.
Work collaboratively with individuals, families, carers, communities and other professionals to ensure the best outcomes are reached for individuals. 19. To comply with all Divisional, Departmental and Council Policies and Procedures. 20.
Report and accidents/complaints/untoward incidents to the management team in a timely fashion in accordance with existing policies and procedures 21. To report any incident of ill treatment to a service user, to the Manager and, if not satisfied with appropriateness of the action taken, to Senior Management. 22. To participate in the operation of the Councils Appraisal Scheme.
23.
