WAGE RANGE: $22.50 - $26.30/hourly
JOB SUMMARY: Catholic Charities is seeking a self-motivated individual who is dedicated, resourceful, compassionate, trauma-informed, and culturally aware to serve as a Recuperative Care Coordinator at its Endeavors location.
In coordination with Hennepin County’s Healthcare for the Homeless Team, Catholic Charities’ Recuperative Care Program provides shelter, food, healthcare, and support services to people experiencing homelessness who are unable to recover from a physical illness when staying in a shelter or outside and are being discharged from a hospital or needing to stabilize to receive medical care.
The Recuperative Care Coordinator will be the primary point of contact for the Recuperative Care Team from Healthcare for the Homeless and will be responsible for coordinating all aspects of the Recuperative Care program and relationship. Will conduct intakes, wellness checks and maintain program and property management requirements while providing general case management to the clients in the Recuperative Care program.
MINIMUM QUALIFICATIONS/EXPERTISE:
- Bachelor’s degree in human services or related field preferred with minimum of two (2) years of experience working with a similar client group in property or program services.
- In lieu of degree, a minimum of three (3) years of experience working with a similar client group in property or program services.
- Additional training, education, or licensure may be required per program need.
- Has ability to work with diverse and/or low-income populations, including those who may be experiencing homelessness, unstable housing, and/or mental illness.

Responsibilities:
- Primary Catholic Charities contact with the Recuperative Care Team from Healthcare for the Homeless.
- Maintains a positive working relationship with the Healthcare for the Homeless team and produces regular reports on program utilization, unit turn times, and any program issues.
- Facilitates the housing support for program clients including room assignments and intake, housing assessments, and providing general housing information.
- Coordinates Property /living arrangements including property storage, room key replacement, conducts daily welfare checks.
- Facilitates entry and exit from program/facility.
- Supports and advocates on behalf of client in the procurement of services and benefits entitlement, assisting with required paperwork as necessary.
- Maintains complete case documentation and records, as required by Catholic Charities, the funders, or licensure/regulatory bodies.
- Coordinates with the program and property supervisors on all aspects of client care and seeks appropriate guidance and feedback from other team members.
- Other duties as assigned.
