ABOUT THE ROLE | HE KŌRERO WHAKAMĀRAMA MŌ TĒNEI TŪRANGA
You will be working as part of our CREST Nursing service as a Case Manager to provide personalised community-based assessments to newly discharged clients with a restorative approach. This involves identifying and resolving issues as and when they arise to maximise rehabilitation potential. This short term service aims to return clients back to their baseline level of function and if that is not possible, to plan for long term support.
This role is about working with our already established team of Nurses, Coordinators, Support Workers and other Allied Health within the community setting. As a Registered Nurse you will provide a broad range of care to clients in their own home such as:
- Passion to deliver exceptional services to people with health or injury related needs in their own homes.
- Demonstrated experience completing assessments
- Ability to develop and review goal focused service plans based on individual client needs
- Excellent customer service
- Ability to work autonomously and as part of a team
This is a full-time role, which will mainly be Monday to Friday. Part of the role is providing a 6 day per week service to clients which includes nurses working some weekends on a rostered basis, and on call on some evenings.
At HealthCare NZ, we’re passionate about creating an inclusive workplace that promotes and values diversity, reflecting the communities we serve. If you would like to work in a role that is both rewarding and challenging and would like to make a difference, then we would like to hear from you!
