The post holder will be based in The Anticoagulation Service and will work with the anticoagulation nurse specialists to providing education and counselling to patients who are starting prophylactic or therapeutic anticoagulation working under the direct supervision of the
senior Anticoagulation nurse specialist .
The Anticoagulation service runs the outpatient DVT service; the DVT service involves assessment of patients referred for possible DVT and it provides the initial patient counselling/education session following such a diagnosis and refers this patient group to the
Anticoagulation service and Haematology Consultants for ongoing anticoagulation management.
The post holder will act as a role model for the anticoagulation service, participating in and supporting the ongoing development of clinical practice, service development, audit and research to maintain standards of care.
To promote high quality clinical care and assist in the development of the Anticoagulant Service by ensuring the use of evidence-based practice and excellent clinical standards within the speciality.
To be responsible for the on-going assessment of the quality of patient care; including evaluation of own practice.
To ensure that patient records (hospital notes and computer
software) are maintained to the highest possible standard adhering to information governance guidance.
