Responsibilities:
ABOUT THE ROLE
Enable the provision of high-quality client and carer focused palliative care services across the continuum of the care settings, but most particularly in the home-based setting.
This will occur within the principles of care coordination where clients have access to highly coordinated health care and community support systems.
The Community Palliative Care Nurse is an important member of the palliative care service team operating in a complex environment characterized by a need to;
- Work as part of a multidisciplinary team that supports palliative care service delivery predominantly in the home but also across the care continuum.
- Provide leadership and advocacy to palliative care clients across the care continuum.
- Balance competing priorities and work demands and identify and respond appropriately to critical and urgent clinical needs.
- Support the Clinical Nurse Consultant – Palliative Care, in appropriately prioritizing client need and to operate efficiently and effectively with a view to meeting community needs and providing a financially sustainable service.
- Understand and operate in a work environment that needs to meet clinical quality and safety guidelines
- Work closely and in collaboration with internal and external stakeholders.
DUTIES
- Provide effective day to day clinical and emotional support to palliative care clients and their caregivers.
- Establish, maintain and adapt effective therapeutic partnerships with individuals and their caregivers and family that are person-centred and therefore address the individual’s needs, circumstances and preferences.
- Negotiate mutually agreed goals of care and facilitate person-centred decision making and advanced care planning to promote optimal outcomes for individuals, their caregivers and family.
- Recognise the effects that the intimate and intense nature of caring for individuals, their caregivers and families can have on self and other members of the team, and respond effectively.
- Support the Clinical Nurse Consultant – Palliative Care to build the capacity of nurses, other health team members and the wider community to understand and respond to complex palliative care health and support needs for individuals, their caregivers, and family.
