Responsibilities:

THE ROLE

At Frederic House, we are committed to providing high-quality nursing care in an aged care facility to individuals facing drug and alcohol addiction and/or mental health issues and are at risk of experiencing homelessness. Our team is dedicated to making a positive impact in the lives of our clients, and we strive to create a safe and supportive environment for their recovery.
In this rewarding role you will be coordinating and delivering high quality nursing care to Residents of Frederic house who are experiencing alcohol and other drug dependence and / or mental health issues and are at risk of experiencing homelessness.

RESPONSIBILITIES

  • Provide health care services to individuals facing drug and alcohol addiction and/or mental health issues and are at risk of experiencing homelessness.
  • Liaise with prescribing medical practitioners and chemists.
  • Manage and monitor drug and health records of clients.
  • Conduct holistic nursing care in accordance with the ANMC National Competency Standards.
  • Form therapeutic relationships with clients.
  • Communicate effectively with clients, staff, and external agencies.
  • Contribute to implementation of the Aged Care Quality Standards.
  • Participate in the multidisciplinary care planning and clinical review processes with internal and external teams.
  • Actively promote the integration of clients into mainstream community resources.
