Description:

An exciting opportunity has arisen within the addictions service – we are seeking a Nursing Associate who will take the responsibility of being a supervised primary nurse as part of a multi-disciplinary team supporting a complex patient group in their treatment and recovery journey.
The post holder will work out of the East Riding Partnership “Central Hub” based at Baker Street, Hull. They will provide a seamless service in a multi-agency and multi-disciplinary approach to substance misuse treatment and supporting services within Hull.
Working across primary care, this service supports people and their families by providing long term support to improve their health and wellbeing and reduce the incidence of long term health conditions associated with drug and alcohol use.
The aim of this role is to support the nursing staff and other health professionals in giving quality, patient centred care. This may be physical health observations and procedures, screening, health promotion, opioid substitution treatment and connecting with the wider community to achieve a meaningful recovery.
You will work with the clinical teams within the substance misuse treatment delivery system and role model. This will include physical health and baseline observations, blood borne virus screening, generic assessments, assessing and managing risk, co-working, care planning and reviewing, and assertively working with patients/individuals to support and maintain their recovery.
Those people in the service have a number of protective factors or co-morbid health conditions, which make achieving abstinence from drug and alcohol particularly challenging. So clinical recovery is primarily focused on maintaining stability.
Where individual circumstances change the team prepare and support people to become drug and alcohol free.
Each person has a named case manager who is their primary contact for agreeing their care plan, and co-ordinating joint working with other services such as housing, social care and mainstream mental and physical health.
You will be supporting and inputting information at the Multi-Disciplinary Team, feeding back to the patient the outcome. You will also be integral in signposting patients to relevant services and assisting your colleagues to get the best recovery outcomes.
