We provide rehabilitation in the patients’ home environment with a focus on promoting independence, meeting rehabilitation goals and reducing care needs in the community. Rehabilitation assistants are integral to running our patient groups in Southall and Acton.
This is an exciting opportunity for a health care support worker who already has some experience of working with patients in an acute or community healthcare setting to expand their skills and work within a friendly community team.

To assist the therapists by carrying out delegated aspects of patient care such as practising exercises or reviewing equipment, with supervision as required, reporting back patient progress and informing qualified staff of any issues.
To assist with equipment ordering and stock control for department supplies and ensuring that these are received in full and good working order, following up on any outstanding items or actions as required.
To undertake support work to aid the rehabilitation of patients in the community or the management of patients at home to prevent and unnecessary hospital admission.
To assist with running of various therapist lead group exercise sessions such as upper limb exercise class, Falls, balance & strength class and adapted tai chi exercise class.
