Description:

Are you passionate about working with older adults in a rehabilitation setting with a range of physical and mental health needs?
Are you able to work holistically in a patient-centred environment?
We are looking to recruit a Rehabilitation Support Worker to join our therapy team at Lings Bar Hospital. The focus of the hospital is to maximise and enable independence of the individual and the nature of work is fast paced.

Responsibilities:

To support the clinical staff with the rehabilitation and discharge planning of patients.
To work independently providing delegated therapeutic interventions both on the wards and in the patients own home.
To lead group rehabilitation, education and exercise sessions.
To support the management of equipment provision and contribute to the MDT management of patients.
To be able to work flexibly across ward.
