Contract: Permanent Full-Time Position (Hybrid)
Working Hours: 37.5 Hours
Work Pattern: Monday – Friday (8.30 am – 6.00 pm flexible 37.5 hours)
Salary Range: to be discussed depending on experience

ESSENTIAL REQUIREMENTS

  • Must be a registered general nurse with the NMBI.
  • Minimum of 3 years of clinical experience in a relevant clinical area.
  • Excellent interpersonal, communication, and telephone skills.
  • Ability to work as part of a team while also demonstrating leadership skills.
  • Strong IT skills with a willingness to develop the same.
  • Commitment to courteous, sincere, and sensitive patient service.
  • Excellent organisational skills.

DESIRABLE REQUIREMENTS

  • Experience in chronic disease management in heart failure, COPD, diabetes, etc with postgraduate qualifications an advantage.
  • Clinical remote monitoring experience or experience working with patients remotely using digital technology.

Our Care-Connect Monitoring Nurses are vital to this initiative as they provide direct care to patients. We are now looking for an approachable, committed, and professional Registered General Nurse to join our team. We are seeking someone who can deliver a courteous, sincere, and empathetic patient service remotely, whilst also demonstrating excellent clinical care. Successful candidates will join the Care-Connect team to provide remote clinical monitoring to patients with a range of chronic conditions such as heart failure and COPD. Remote patient monitoring is provided using a specially designed patient app and cases are managed using a specially designed case management system. The Care-Connect clinical team provides support and guidance to patients particularly when a change in clinical indicators is identified or there is a deterioration in their symptoms. All clinical care is provided remotely to patients by the Care-Connect team using a range of digital technologies. The Care-Connect nursing team reports to the Clinical Lead Nurse who is supported by the Care-Connect Medical Director. This is a hybrid role working out of the RSA House, Sandyford Rd, Dundrum, Dublin, D16 FC92, days to be agreed.

ROLE & RESPONSIBILITIES

Deliver safe, effective, high-quality, and compassionate remote patient care.
Responsible for the monitoring and assessment of clinical measurements and symptom questionnaire responses, using our specially designed monitoring platform and case management system.
Working autonomously, using high-level clinical decision-making skills, to evaluate clinical data efficiently and safely.
Identify clinical changes or deteriorations in a patient’s condition using available data and follow the appropriate clinical escalation pathways as required.
Provide clear and confident guidance and support to patients as required.
Maintain detailed and clear clinical records and ensure all data collection is completed as per the standard processes.
Maintain the highest standards of patient communication and care to ensure patients’ health and well-being are maximised to their optimal potential.
Work closely with fellow nurses and the multi-disciplinary team (MDT) while promoting and ensuring effective communication and good relationships with other team members and patients. Continuously engage with and provide feedback to the MDT to ensure continuous development and improvement of the programme.
Identify and respect the needs of each patient and team member while providing efficient and high-quality care.
Work closely with the Care Coordinator team to ensure patients are onboarded onto the programme quickly and efficiently and to manage all non-clinical patient communications.
Support family members who are supporting their families in participating in the programme.
Ensure that the highest standards of patient confidentiality are maintained in relation to patients and patients’ records.
Make every contact count; educate the patient and their family using up-to-date, evidence-based guidelines, and policies.
Participate, and where requested lead, in clinical audits and ensure continuous monitoring of the service.
Support, develop, and implement required identified changes and improvements to the service.
