Description:
Governance Organise and contribute to the structure of the monthly team meetings, including identifying themes and trends to be included as well as supporting producing agendas, and taking minutes. Support the Service Lines with information, reviews of data to achieve improvements in practice/service development and to support the evaluation process. Network across the Service Lines to support and respond to patient safety, inquests, and learning. Support and liaise with portfolio leads in the set up and facilitation of Learning Responses, such as After Action Reviews.
Support the production of regular patient safety and governance reports as required. Take responsibility for keeping up to date with governance requirements, locally and nationally. Patient Safety/Inquest To support the Inquest Lead with the inquest administrative process such as logging new inquests, creating folders and using Ulysses to support data requests such as Witness Statements. Monitor the systems in place to ensure tracking of inquests, outcomes and any action plans to support learning and improvement and report trends to appropriate portfolio leads as required.
Provide immediate advice and support to staff in reporting patient safety incidents and accessing reports or data extractions in relation to inquests. To be able to deal with and process sensitive, confidential and sometimes distressing information as required. Analysis and Reporting Review and analyse data relating to complaints, patient safety reporting and patient experience. Provide written and verbal reports for the Governance Team and support the response to Freedom of Information Requests (FOIs).
Produce performance data information for the groups within the Service Lines that is both qualitative and quantitative in support of clinical governance activity. Compliance To support the Compliance Manager at the time of CQC inspections or other compliance activities when appropriate. Risk Management System (Ulysses) Support the Risk Systems Officer in the facilitation and delivery of the Ulysses Safeguard system and process. To quality control and support on a daily basis the management of all aspects of the Learning from Patient Safety Events (LFPSE) tasks, such as reviewing data quality, managing errors, uploading and merging Patient Safety Incidents.
Review and assess Patient Safety Incidents that require escalation to the Learning and Response Manager. To provide administration and technical support for the maintenance and future development of the Ulysses Safeguard System. To support the Risk Systems Officer in the timely production of ad-hoc reports and to analyse complex data as required, for example: Freedom of Information (FOI) requests. To support the Ulysses Helpdesk function responding to all enquiries within specified timeframes.
Prioritising as appropriate. To provide administrative cover and support in times of annual leave and sickness absence to the Quality Governance Team (Patient Experience, Patient Safety, Compliance and other administrative roles). Ensuring that all tasks are prioritised accordingly, such as supporting the PALS telephone line and supporting Compliance/Quality Account activity. To maintain and promote a strict code of confidentiality in all business dealings, maintaining the integrity and security of the Ulysses Safeguard System in line with Trust data quality standards and to behave professionally at all times.
To implement and contribute to policy and service development in relation to own area as required. To be responsible for maintaining the Quality Governance inSite pages, including incidents, claims and inquests
