Several exciting opportunities have opened up for aspiring RMN’s to join our teams in the community in the heart of Brent where we have various opportunities to help you become a more enhanced community practitioner .
We are looking for focused individuals, who are highly motivated with excellent communication skills. Community Services are becoming fast paced and rapidly changing to support the ‘Out of Hospital Care’ strategies, you will therefore need to demonstrate resilience in the ability to adapt to the changing landscape and opportunities.
You will be expected to case manage an allocation of patients, monitor patients’ mental state, promote independence, promote social inclusion, aid the recovery process, and reduce hospital admissions
Positions available to you;
CMHT- Monday to Friday 9am till 5pm, however we are open to considering flexible working opportunities including condensed hours and working from home where possible to provide you with a healthier work-life balance
