Department Overview:
The Nurse Navigator will work collaboratively with the faculty experts in Hematologic Malignancies. The NN also works with Knight Cancer teams to include providers from hem medical oncology, surgical oncology, surgery, radiation oncology as well as RNC, CC, and TC for the care of hematologic malignancy patient population. The nurse navigator coordinates the care for the patient prior to the patient’s first consult visit at OHSU, and in some cases, through the work up and diagnosis phase of care. This position will work with the clinical nurse manager and other ambulatory oncology nurse navigators to develop standard metrics, identify patient barriers, coordinate smooth transitions of care, and to be available for team members in gathering data for new patients that will allow the team to create a plan of care at the patient’s first visit and, through the work up phase of their disease. This position is part of the hematology/hem malignancy cluster within the Nurse Navigator team and provides cross coverage as needed for navigating patients for new consults with disease expertise in leukemia, lymphoma, myeloma, transplant, CAR-T therapy.
An Oncology Nurse Navigator (ONN) at OHSU is a professional RN with oncology-specific clinical knowledge who offers individual assistance to patients, families, and caregivers to help overcome healthcare system barriers. Using the nursing process, an ONN provides education and resources to facilitate informed decision-making and timely access to quality health and psychosocial care as they approach their first visit. This individual will serve as the primary point of contact for all navigated patients within the disease specialties they cover and will thoroughly hand off care (verbally and in EMR documentation) to providers and nurses assuming care of the patient after the first consult.
The ONN interfaces with research staff, and works to identify patients who may be suitable candidates for clinical trials.
Function/Duties of Position:

$49.16-$78.18 PER HOUR, BASED ON RN DEGREE AND LEVEL OF EXPERIENCE

Required Qualifications:

  • BSN Graduates: Baccalaureate Degree in Nursing from a program accredited by Commission of Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN) or Commission for Nursing Education Accreditation (CNEA) 30 days before start date.
  • ADN Graduates: Associate Degree in Nursing from an accredited program 30 days before the start date.
  • Associate degree Nurses required to enroll in BSN program within 3 years of hire and complete within 5 years of hire
  • Internal ONA applicants: Associate Degree in Nursing; BSN preferred
  • Current, unencumbered Oregon State Registered Nurse License
  • BLS at time of hire and must be issued by the American Heart Association (AHA)
  • BLS must not expire during orientation
  • Must be able to perform the essential functions of the position with or without accommodatio

Shift 8 hrs M-F; eight hour shifts to include the 0800. Negotiable in alignment with needs of the specialty team’s needs. Intermittent exposure to usual hospital environmental conditions including possible exposure to blood and body fluids for which staff is provided education and protective equipment to minimize risk. This position may be eligible for partial remote work after orientation and as deemed feasible by the rest of the interdisciplinary team. On campus work is required a minimum of one full shift per week.

Responsibilities:

  • The ONN thoroughly reviews clinical information (records, diagnostic test results, etc.) submitted with the referral reviewing for urgency, other needed tests, potential eligibility for clinical trial and to identify which physician/s best suited for consult. The ONN summarizes this interaction which via a signed navigation assessment note in Epic which serves as a thorough hand off to the physician and clinical team/s that will consult and/or follow the patient. Additional documentation or communications may be required as determined by the team such as e-mail or in clinic visit with the patient.
  • Conducts a patient assessment interview to identify patient’s understanding of condition, barriers to care, social determinants of health, preferred learning style, family support, and any current symptoms. Arranges for first outpatient consult, or if patient’s acuity requires it, works with clinical team to arrange for direct admit and diagnostic work up in hospital (i.e., bone marrow biopsy).
  • The ONN tracks data on each referral in order to capture information on the population served and to follow metrics for volume, access, referral sources, diagnostic work up requirements, subsequent needs after consult.
  • The ONN may meet with new patients in clinic or on the inpatient unit as determined by the specialty team.
  • The ONN may follow the patient through the diagnostic work up and also assist with transitions of care for the patient between treatment modalities. This is particularly true in specialties with multi-disciplinary clinics.
  • The ONN ensures that when hospitalization is required; those patients and their family members understand prior to admission, what they can expect during the pre-hospitalization, hospital stay and post discharge period. The ONN coordinates with RN Coordinator and the care coordination team and inpatient team on the admission and discharge plan.
