Description:

Provide clear and effective leadership and co-ordination of the patient pathway for children with medical complexity (CMC). Work with minimal supervision to develop and deliver the specialist service based on principles that are evidence based and which enhance the quality of patient care. Ensure a high-quality service meeting the individual needs of the child and their family. Lead the co-ordination of holistic and seamless clinical care for CMC, who are often known to several specialty teams.
Promote efficient patient pathways Work collaboratively with all stakeholders including MDTs, CNSs, clinical teams, Consultants, therapists, and community nursing teams across the Trust/Health Board to facilitate improved patient flow, capacity and continuity of care. Effectively liaise with education and social care to reduce fragmentation of care across organisational boundaries. Co-ordinate multidisciplinary assessments meeting the physical, psychological, emotional and spiritual needs of the CYP and their family at times of stability and crisis and together with the multidisciplinary team develop strategies to address these needs. Prepare and participate in patient reviews and child protection case conferences with team members and other agencies and providing reports as required.
Provide leadership and act as a resource for clinical staff, providing expert advice in care planning for CMC. Work closely with ward-based nursing and medical colleagues from time of admission to provide a co-ordinated, seamless, individualised approach, ensuring that families are fully informed regarding care planning and on-going arrangements to meet their child’s clinical needs. Manage case load of CMC working autonomously while consulting and escalating concerns as needed. Provide evidence-based teaching / training to CYP and their families, providing up-to-date evidence-based verbal and written information / leaflets about their conditions that is age and culturally appropriate to promote and develop their confidence in self-care.
Provide health education for the whole family, incorporating health-promoting strategies within the care plan as appropriate ensuring that the family/carers have a suitable level of information, preparation and training. Assist families to network with others in similar circumstances/ access support groups to reduce feelings of isolation. Monitor and support attendance at clinics to reduce DNA (Did not Attend) rates. Co-ordinate clinic appointments to reduce number of visits.
Provide accessible advice and practical support, including teaching when required, to relatives of CMC being a contact point for families throughout the patients pathway, providing evidence-based information to patients and their families as required. Lead and drive safe reduction in the length of stay for inpatients by being responsible for timely intervention and appropriate escalation in the prevention of avoidable discharge delay. Assist in development of clinical standards, protocols, shared documentation, and training which support clinical decision making. Ensure the use of electronic passports.
Collect data and compile accurate and up to date reports for both internal and external partners including Roald Dahl Marvellous Childrens Charity. Act as Ambassador for Roald Dahl Marvellous Childrens Charity, raising awareness of the charity amongst colleagues and families
