The Opportunity – We are looking to hire a Senior Case Manager to join our growing team at our outpatient substance abuse treatment facility. The Senior Case Manager’s responsibilities include overseeing the overall daily case management functions to ensure clients in our outpatient program receive quality client-centered care.
To be successful as a Senior Case Manager, you must have strong written communication skills, proficiency with taking notes, ability to multi-task, and building relationships with others. This role is a vital member of the Good Landing Recovery System of Care and will be expected to provide clients with a positive experience by providing a process of assessment, planning, and facilitation of care coordination, and advocate for options and services to meet an individual’s need through clear communication and available resources.

REQUIRED SKILLS/ABILITIES-

  • Excellent verbal and written communication skills
  • Excellent organizational skills and attention to detail
  • Familiarity with EMR software (Kipu, BestNotes, etc.)
  • Flexible Schedule

  • Excellent time management skills with a proven ability to meet deadlines
  • Ability to function well in a high-paced and at times stressful environment
  • Proficient in all Microsoft Office applications as well as medical office software

EDUCATION AND EXPERIENCE-

  • High school diploma or equivalent
  • Associates degree in Business, Healthcare Administration, or related field (preferred)
  • At least 1-2 years of related substance abuse and mental health experience
  • If in recovery, a minimum of 1 year sober

Responsibilities:

  • Reports directly to the Director of Operations.
  • Provides quality casework services to clients in the areas of crisis intervention, stabilization, goal planning, community resources, and necessary court proceedings.
  • Identifies social, economic, and physical needs and barriers with clients by assessing client’s support systems, available community resources, and other factors to develop an appropriate case plan.
  • Collaborate with clinicians regarding behavioral interventions and assistance for clients throughout their duration of their episode of care.
  • Maintains all required documentation, records, and reports.
  • Facilitates community referral and follow-up with, on, or behalf of the client.
  • Explains facility and program guidelines, regulations, and procedures by assisting clients in completing required forms, and gathering necessary documentation.
  • Ensures compliance with all federal, state, and local department laws, regulations, and requirements.
  • Responsible for providing various ecclesiastical duties in support of the company and company owner’s core beliefs to include, but not limited to, providing prayer to clients, and providing practical biblical guidance.
  • Performs other related duties as assigned.
