Responsibilities:
This Grade 3 Senior Clinician Dietitian role will be working within the multidisciplinary subacute At Home Subacute service to achieve optimal rehabilitation outcomes for people in their homes and community settings. There is a strong emphasis on providing excellent evidenced based practice treatment, with a multidisciplinary team approach for person centered care. Care will be delivered within neurological, orthopaedic, general rehabilitation, GEM, cancer care and palliative care streams. This dietitian role will also provide service to the Hospital in the Home stream, including Home Based Cancer Care.
